Your First Appointment

Don’t forget to bring the following to your first appointment with Dr. Aramini:

• Medical Insurance Card
• Any previous x-rays or MRIs if applicable
• If you have an HMO policy, be sure to have a referral from your primary care physician
• Please fill out and bring these patient forms

Payment Requirements

If you have a co-payment, please plan on paying this at your time of visit. You may be responsible for paying a deductible, depending on the services and your plan. Check with your insurance company prior to visiting regarding any questions about your coverage.

If you do not have medical insurance, please inform us of this at the time you make your appointment.
